Request Account/ Data Deletion

I consent to the deletion of my Connected Mind account and associated data. I understand that my account will not be deleted if there are any active subscriptions. I understand that I may cancel my subscriptions at any time in my Secure Portal at https://secure.connectedmind.me. I understand that my account cannot be retrieved after its deletion. I understand that the deletion of my account may take up to 30 days. I understand that my account may exist in archived backups for up to 1 year. I understand that additional verification of my identity may be required before my account is deleted. I understand that if my identity cannot be verified my account will not be deleted. I understand that Connected Mind, Inc will retain a de-identified copy of any mental health screening/ testing performed using my account. I understand that Connected Mind, Inc will not retain any Protected Health Information (PHI) related to my account. I understand that Connected Mind, Inc will abide by the Business Associates Agreement entered into during my account creation.
