Multi-Condition Screening at Intake
Connected Mind® with Fast Check® gives practices one starting point for multi-condition screening at intake. Eight initial questions screen for depression, anxiety, ADHD, bipolar disorder, substance use disorder, and somatic symptom disorder. When results indicate further testing, the corresponding Standardized Assessment Modules (SAMs) are administered automatically.
The initial screen takes approximately one minute. Follow-up testing takes additional time.
Look beyond a single presenting concern
A patient may bring more than one mental health concern to a visit. A broad initial screen gives the clinician structured information across several conditions before deciding what to explore next.
The published Connected Mind Fast Check (CMFC) study evaluated both the initial screening and follow-up SAMs in primary care. Its authors discussed patients who were unaware of their existing conditions and co-occurring concerns. Read the peer-reviewed study or explore our clinical evidence.
A practical intake workflow
1. Choose when patients complete the screen
Use a clinic tablet for in-office screening and testing, or send a secure remote link for completion before or during a telemedicine visit. Choose the point in your intake workflow that gives the care team time to review the results.
2. Start with the same broad screen
The eight initial questions screen for six mental health conditions in one workflow. Staff begin with Connected Mind rather than selecting a separate starting screen for each suspected condition.
3. Let indicated testing follow automatically
When initial results indicate further testing, Connected Mind administers its own corresponding SAMs. More than one condition can lead to follow-up testing in the same encounter.
This is a screening-and-testing sequence: the approximately one-minute starting screen and the additional SAMs are separate stages.
4. Put the results in front of the clinician
A qualified provider independently reviews the structured results and makes clinical decisions. Reports bring together condition-specific results and longitudinal history.
Your practice can receive reports through the Direct messaging your EHR already uses. It controls which reports are included, whether they arrive combined or separately, and the report formats. PDF, HTML, and LOINC-coded results are delivered in a C-CDA document your EHR can read. Explore EHR integration.
Give each role a clear part in the process
Providers, medical assistants, front-desk staff, billers, and administrators have different responsibilities. Connected Mind provides role-specific training, with courses taking 15–20 minutes per role. The portal tracks training progress, assessment results, and certification status.
The Connected Mind Support Assistant helps staff with product, training, workflow, and billing questions and helps them find the right portal resources. View training by role.

How long does screening take at intake?
The initial eight-question screen takes approximately one minute. Indicated SAMs take additional time. Practices can offer screening on a clinic tablet or use a secure link for remote completion.
What happens if more than one condition needs further testing?
Connected Mind automatically administers the corresponding SAMs for the conditions indicated by the initial results. Clinicians receive structured results for independent review.
Can patients complete screening before a remote visit?
Yes. Practices can send a secure link for patients to complete screening and indicated testing remotely before or during a telemedicine visit.
How do results fit into the practice workflow?
Clinicians can review results in the Connected Mind portal. Reports can also be delivered automatically through the practice’s Direct messaging connection, using its selected report content, grouping, and delivery formats.
